What is the NHS blood pressure check service?

The NHS Hypertension Case-Finding Service is a free check offered at community pharmacies across England. We measure your blood pressure using a calibrated monitor and explain what your reading means in plain English. The aim is to spot raised blood pressure early, before it has the chance to cause problems.

If your reading at the pharmacy is high, we may offer ambulatory blood pressure monitoring (ABPM). This involves wearing a small monitor for 24 hours, which gives a more accurate picture of your blood pressure during everyday life. The service is free at the point of use and you do not need to be registered with us as a patient.

Who is it for?

High blood pressure rarely causes symptoms, so a simple check is the only reliable way to know whether yours is in a healthy range. The NHS service is aimed at adults who have not already been diagnosed with high blood pressure, with a particular focus on people aged 40 and over.

You may also benefit from a check at a younger age if you have a family history of heart disease, stroke, or high blood pressure, or if you have other risk factors such as being overweight, smoking, or high stress levels.

This free NHS check is suitable if you:

  • Are aged 40 or over and not already diagnosed with high blood pressure
  • Have a family history of heart disease or stroke
  • Have not had your blood pressure checked recently
  • Want a baseline reading for peace of mind
  • Have been advised by a clinician to monitor your blood pressure
  • Are under 40 with risk factors and have been referred by a healthcare professional

If you already take medication for high blood pressure, please continue to follow the monitoring plan agreed with your GP.

What to expect at your appointment

Your appointment takes place in our private consultation room. We will ask a few short questions about your health and any medication you take, then measure your blood pressure using an upper-arm cuff. We usually take more than one reading, with a short rest in between, to make sure the result is reliable.

We will explain your reading there and then. If it is in the normal range, we will share simple lifestyle advice and let you know when to check again. If it is raised, we may offer 24-hour monitoring or refer you back to your GP, with a copy of your results, so any next steps can be arranged quickly.

Getting an accurate reading

A few small things can affect your reading on the day. Caffeine, nicotine, exercise, and a full bladder can all push your blood pressure up temporarily. To help us get the most accurate measurement, please try to follow the simple steps below before your appointment.

If your reading is higher than expected, try not to worry. A single high reading does not mean you have high blood pressure. We will talk you through what it means and the sensible next steps.

For the most accurate result, in the 30 minutes before your appointment please try to:

  • Avoid caffeine, including tea, coffee, and energy drinks
  • Avoid smoking or vaping
  • Avoid heavy exercise
  • Empty your bladder before you sit down with us
  • Wear loose clothing that lets us roll a sleeve above the elbow
  • Arrive a few minutes early so you can sit and relax

After your check

We will give you a written copy of your reading and explain what it means. If everything looks fine, we will suggest how often to check again, which for many adults is every five years.

If your reading is raised, we will either fit you with a 24-hour monitor or refer you back to your GP with your results. Your GP can then decide whether further tests, lifestyle advice, or treatment are needed. Most people with high blood pressure manage it well with a combination of healthy habits and, where appropriate, medication.
